A printable IBD symptom log, one week per page

A paper log for Crohn’s or ulcerative colitis. Each column is a day, each row is something to jot down: BMs, blood, urgency, pain, stress, sleep, meals and anything else. Print a few, keep one by the bed or in your bag, and fill in what you want to.

Prints on one page, US Letter or A4.
How people use it
  1. Print a week

Print one sheet per week. It fits US Letter and A4.

  2. Jot it down

Fill in a column each day, or a few times a day. Leave anything blank you don’t want to track.

  3. Bring it along

Take the last few weeks with you when you talk with your care team, so the numbers are in front of you.

Here’s what prints

What’s on the sheet

Each row is a plain label with a simple scale or a blank box, so you can write it your way:

  • Bowel movements: how many. A tally mark each time is the easiest way for a lot of people.
  • Blood: circle Y or N.
  • Urgency: circle 0 to 3, where 0 is none.
  • Pain: 0 to 10, where 0 is none. Stress: 1 to 10.
  • Sleep: roughly how many hours, and a word for how it felt.
  • Meals and other notes: whatever you want to remember about the day.

The days are numbered rather than named, so you can start on any day of the week. If you want ideas for what else to note down, our guide on what to log each day with IBD goes through it, and the Bristol stool chart guide explains the stool types if you like to note those in the margin.

The sheet doesn’t score anything or tell you what your numbers mean. It’s a record of your days. For what they mean for you, talk with your care team, and take a few weeks of sheets along with our care team visit sheet.

How Flarely helps

Prefer your phone? In Flarely you log a BM with blood, urgency and pain in three taps, stress in one, and a meal by snapping a photo. The Summary tab lays out your BMs, blood and urgency by day, with charts for 7, 30 or 90 days.

When you want to share it, Flarely makes a 30-day summary PDF in one tap for whoever helps you: family, a friend, or your care team.
